Get paid to care for a family member in Wisconsin
In Wisconsin, DHS says IRIS participants can hire family members as personal care workers. Whether Family Care managed care organizations can pay relatives is not stated on the DHS pages we read.
In Wisconsin, DHS says IRIS participants who use Self-Directed Personal Care may hire family members to provide their care. The person who represents the participant cannot also be the paid worker. The DHS pages we read do not say whether spouses or parents of minors can be paid.
The route for family
Wisconsin has no separate family caregiver payment program on the pages we read. The route is IRIS, the self-directed Medicaid long-term care program. DHS's page on IRIS Self-Directed Personal Care, last revised April 16, 2026, says you can hire, train and oversee your own personal care workers, that an IRIS nurse builds your personal care plan with you and a doctor, and that you may also hire family members.
In Family Care, a managed care organization arranges services through its network. The DHS pages we read do not say whether an MCO can pay a relative. Ask the MCO in writing before you plan around it.
Who has limits
- Your representative. DHS says whoever represents you in IRIS Self-Directed Personal Care cannot also provide your personal care.
- Spouses, parents of minors, guardians. Not stated on the pages we read. We do not say yes or no.
- Workers who skip visit verification. The rule is about the participant: DHS's EVV policy says failing the EVV requirements results in disenrollment from IRIS.
Electronic visit verification
DHS's IRIS policy, 11/2025, says personal care workers who are not live-in must use electronic visit verification. It records who receives the service, who gives it, what, where, the date and the worker's start and end times. DHS provides a free system called Sandata, and fiscal employer agencies may use another certified system. The participant, as the employer, must make sure workers use it.
A worker counts as live-in if they permanently live in the same residence as the participant, or permanently live in the other half of a two-residence dwelling and are a relative. DHS asks for proof such as a Wisconsin driver's license or a lease. Live-in workers do not have to use EVV.
Pay and hours
The DHS pages we read give no wage or rate. IRIS works within a budget, and the fiscal employer agent handles payroll, according to DHS. Ask your IRIS consultant what rate and hours are possible.
Steps
- Contact your local aging and disability resource center (ADRC) to check eligibility for IRIS.
- Ask the IRIS consultant agency whether your relative can be hired, for which services, and whether they would count as live-in for EVV.
- If personal care is in your plan, work with the IRIS SDPC nurse and the oversight agency, listed at 844-747-7372.

Common questions
Can I get paid to take care of my parent in Wisconsin?
Possibly. DHS says IRIS participants who use Self-Directed Personal Care may hire family members to provide their care. Your parent would need to be enrolled in IRIS and have personal care in their plan.
Can a spouse or a parent of a minor be paid?
The DHS pages we read do not say, so we do not say. Ask your IRIS consultant agency.
Can the person who represents me also be my paid worker?
No, in IRIS Self-Directed Personal Care. DHS says whoever represents you in that program cannot also provide your personal care.
Can a Family Care managed care organization pay a relative?
The DHS Family Care pages we read do not say. In Family Care the managed care organization arranges services through its provider network. Ask the MCO in writing.
What is electronic visit verification?
DHS's IRIS policy of 11/2025 says non-live-in participant-hired workers who give personal care must check in and out with an EVV system, and that failing the requirements results in disenrollment. Live-in workers are not required to use it. A relative who permanently lives with the participant may count as live-in.
How much does a paid family caregiver earn?
The DHS pages we read do not state a wage or rate. In IRIS your budget and the fiscal employer agent set what is possible. Ask your IRIS consultant.
Is the pay taxable?
We did not read a DHS tax page, so we do not say. Ask a tax professional.
