Something wrong with a listing?
Provider details come from the CMS Provider Data Catalog and the CMS NPI Registry, and can be out of date between releases. If a listing is wrong, email us with the provider name, its state, and its CMS certification number or NPI if you have one, and we will check it against the source. If the CMS or NPI record itself is wrong, the provider has to correct it with CMS or in NPPES directly.
We cannot help with urgent care or abuse reports.
If someone is in immediate danger, call 911. To report suspected abuse, neglect or exploitation of an older adult or an adult with a disability, contact your state Adult Protective Services. The Eldercare Locator (1-800-677-1116) can point you to the right office. To complain about a nursing home or assisted living residence, contact your state long-term care ombudsman. Carevo is not a regulator and cannot investigate.
