Get paid to care for a family member in Missouri

In Missouri, Consumer Directed Services let you hire your own attendant. The state manual bars spouses and court-appointed guardians or conservators from being paid, and a minor cannot be hired.

A family member can be paid in Missouri through Consumer Directed Services (CDS) unless they are the participant's spouse or a court-appointed guardian or conservator. The Department of Health and Senior Services (DHSS) manual bars those two groups by name. A minor cannot be hired.

The route for family

Missouri has no separate family caregiver program on the pages we read. The route is Consumer Directed Services, in which you hire and direct your own attendant. The participant must qualify for Medicaid home and community based services, including nursing facility level of care, and must be able to direct their own care.

Who is barred, and who is unclear

  • Spouses: barred. The manual says spouses cannot be the paid CDS attendant.
  • Court-appointed guardians and conservators: barred, as legally responsible individuals.
  • Minors: cannot be hired, even with caregiving experience.
  • Agency owners and certain staff: an agency owner or direct employee who does certain office tasks cannot be an attendant for that agency's participants, with a narrow temporary exception.
  • Payees: allowed, though the provider may want more oversight.
  • Parents of minor children and adult children: the page we read does not name them. We do not say yes or no. Ask DSDS.

If you are in the agency-based Personal Care program instead of CDS, a different DHSS page says the aide employed by the agency generally cannot be a parent, sibling, child, spouse, grandparent or grandchild.

What you need to do

  1. Qualify for home and community based services. DHSS's eligibility chapter, revised February 2026, lists an appropriate Medicaid code, nursing facility level of care, and a face-to-face assessment with a person-centered care plan.
  2. Choose CDS and a CDS agency. The agency obtains an employer identification number and uploads it to your record before services start.
  3. Choose your attendant, who must not be barred above.
  4. Do not enroll in both CDS and the Department of Mental Health's Self-Directed Services. The manual says a participant cannot be in both.

Pay and hours

We found no wage or rate on the pages we read. The hours are the hours authorized in the care plan.

Where to start

Carevo is a connection platform and directory. We do not enroll anyone as a paid caregiver, decide hours or pay, or run payroll.

Common questions

Can I get paid to take care of my parent in Missouri?

Possibly. Under Consumer Directed Services the DHSS manual bars only spouses and court-appointed guardians or conservators by name, so an adult child is not named as barred. Your parent must qualify for Medicaid home and community based services first. Confirm with DSDS.

Can I be paid to care for my spouse?

No. The DHSS manual says spouses cannot be the paid CDS attendant.

Can I be paid to care for my minor child?

The DHSS page we read does not say. It bars spouses and court-appointed guardians or conservators, and says a minor cannot be hired as an attendant. Ask DSDS about parents of minor children.

What about agency-based personal care?

A DHSS page on the agency-based Personal Care program says an aide employed by the provider agency generally cannot be a family member, defined as a parent, sibling, child, spouse, grandparent or grandchild. That is a different program from CDS.

Who runs the payroll?

A CDS agency supports the participant as employer. The manual says services cannot begin until the CDS agency obtains an employer identification number for the participant. We did not read a page that describes payroll steps.

How much does a paid family caregiver earn?

The pages we read state no wage or rate. Ask the CDS agency.
